The 1003
What does a 1003 application ask?
The Uniform Residential Loan Application — Fannie Mae Form 1003 — covers the loan and property, the borrower, employment and income, assets and liabilities, declarations, and acknowledgments and demographic information. Our pre-application gathers the first four in plain steps; the rest is completed in your secure system.
- Loan & property — Purpose, property type, occupancy, location and estimated value.
- Borrower information — Name, contact details, co-borrowers, military service, residence history.
- Employment & income — Employers or business, years in line of work, monthly income by type.
- Assets & liabilities — Accounts, other assets, monthly debts and other properties owned.
- Declarations — Occupancy intent, past foreclosure or bankruptcy, lawsuits, other obligations.
- Acknowledgments & demographics — Consents and the government monitoring questions, handled in the lender's secure system.

Security first
Forms run over HTTPS, spam is filtered with an invisible challenge, and sensitive identifiers are never requested in an open form, chat or text. Your privacy policy explains what is collected and why — we provide the clauses.
What does a 1003 application ask?
The Uniform Residential Loan Application (Fannie Mae Form 1003 / Freddie Mac Form 65) asks about the loan and property, each borrower's personal information, employment and income, assets and liabilities, real estate owned, declarations, acknowledgments, and demographic information collected for government monitoring.
Is this demo a real application?
No. It is a working demonstration of the step-by-step experience we build. Nothing you type is sent anywhere, and it is cleared from your browser when you finish.
Why split the application into small steps?
Long single-page forms overwhelm borrowers on phones. Small steps with a progress bar, a back button and saved progress make it more likely they finish — and they can return later without starting over.
Does your form collect Social Security numbers?
Our pre-application collects what is needed to start the conversation. Social Security numbers and full credit authorisations are collected in your company's secure loan origination system or portal, which we link to.
Can the application be pre-filled from a funnel?
Yes. Quiz answers such as goal, timeline, price range and income type carry into the application so the borrower never answers the same question twice.
Where does a completed application go?
To the CRM contact (with a notification to the loan officer) and, where your company allows, into your LOS through its own intake link. Your compliance team decides the routing.
Is the form accessible?
Yes: labelled fields, keyboard navigation, error messages announced to screen readers, 16px inputs so phones don't zoom, and focus moved to each new step.
Can it be in Spanish?
Yes, as part of a Spanish funnel build. Field labels, help text and error messages are translated natively and reviewed.
